RETIREMENT PLAN (401K)
The company matches 75% of each employee’s total contribution up to 5%. You can sign up on Jan. 1, April 1, July 1, or October 1 as long as you have been employed 90 or more days on any one of those four dates.
MEDICAL INSURANCE-BLUECROSS BLUESHIELD of TN
Company pays 80% of employees base plan premium; 40% of dependents portion of the premium.
LIFE INSURANCE
The company pays a $15,000 life insurance policy on your behalf. You have the option of purchasing a $2,500 policy on your spouse, and $1,000 policy per child through company sponsored plan. You also have the option of purchasing voluntary life insurance up to $30,000 on your spouse and $80,000 on yourself without health questions.
SHORT TERM DISABILITY
The plan pays 67% of your base weekly salary for up to 26 weeks. Premium is based on annual salary.
Accurate contributes 50% of premium cost.
Short term disability insurance is a valuable employee benefit.
Following are the details regarding this insurance:
1) The plan will pay 67% of your base weekly wages.
2) Employees will be eligible for benefits after being disabled for 15 days following a non-occupational accident or illness.
3) Maximum benefit duration is 26 weeks.
4) If a disabled employee can work part time, the plan will pay the difference at the 67% rate.
5) In the event that a disabled employee is receiving benefits and dies, his/her beneficiary will receive a check equal to (3) times the deceased employee's weekly wages.
6) There will be a 90 day initial employment for all new employees before they can become eligible to participate in the plan.
DENTAL INSURANCE
$1,200/year coverage; $100 lifetime deductible; employee funded.
VISION INSURANCE
In-Network: Free Eye Exam every 12 months/glasses (in lieu of contacts)every 24 months.
BEREAVEMENT
Three (3) days paid excused absence for death of immediate family member.
HOLIDAYS
All employees receive (7) paid holidays per year. (eligible after 30 days)
